There was recently a story about a man that was killed in an industrial machine whilst he was inside cleaning it. However these trip switches failed, and another employee, who was unaware, turned on the machine when the other worker was inside it. The machine had a large industrial blade inside, which left the man with critical injuries and he later died in hospital.
